Eligibility
With the exception of self-care, all of our services
and entry into residential care requires potential
residents and clients to be assessed by an Aged Care
Assessment Team and have an approved ACCR form.
These can be arranged through your family doctor or
local hospital. These assessments are valid for 12
months.
